Responsible for providing support to providers to improve the effectiveness and efficiencies of provider practices. Up to 10% travel within North Carolina region.
Primary duties may include, but are not limited to:
Provides support for assigned provider contracts.
Obtains and analyzes quality metrics and reports for care opportunities, supports practice implementation of care coordination and care management, identifies action plans for providers to implement to improve cost, quality and the patient experience and participates in design, development, and implementation of community learning forums.
Collaborates with all Advanced Medical Home (AMH) teams including but not limited to: provider collaboration manager, provider clinical liaison, provider contracting and network relations consultants to meet provider and practice service needs.
Actively participates in developing AMH program recommendations and action plans.
Supports the efforts of providers and practice teams related to care delivery interventions resulting in growth towards AMH Tier 3 attestation and ultimately cost of care savings.
Promotes practice transformation towards meeting the goals specified by NC-DHHS.
Provides education for practices in tandem with clinical and quality staff to develop metric, data, and quality improvement.
May participate in various community and multi-department committees to assure effective AMH program implementation as specified by NC-DHHS.
Tracks progress of program and prepares status reports to management or senior management.
Maintains up to date knowledge on AMH, Care Delivery System Redesign and Accountable Care Organizations.
Requires a BA/BS degree and a minimum of 3 years’ experience in data base maintenance related to health care; or any combination of education and experience, which would provide an equivalent background.
Provider environment experience strongly preferred.
- Public health, Medicaid/Medicare, public policy, physician environment experience in practice transformation/quality improvement, ambulatory care setting quality and efficiency metrics, and electronic health records preferred.
